2024 Summer Camp Application

When completing your camp application, please be sure to read each question carefully and answer each question completely and truthfully.

1. Have the following information ready before you apply. Note: you cannot save and return to the application, you MUST apply in one sitting, please allow 15-20 minutes to apply:
   1. Insurance Information (Private insurance, public aid, Medicare, and/or Medicaid numbers.)
   2. Medical History
   3. Emergency Contact, Primary Care Provider Contact, and
   4. References (first time applicants only)
2. Required Paperwork/Follow-Up Forms – After you submit your application, you will receive a confirmation email with the links to upload the following:
   1. 1. Physical form (required)
      2. Photo of your insurance card (required)
3. Optional Paperwork/Follow-Up Forms – After you submit your application, you will receive a confirmation email with the links to upload the following:
   1. 1. Covid Vaccination Card (Optional)
      2. Medication list (Optional, Camper/Participants only)
      3. For any other documents you wish to provide, such as Power of Attorney information, or Living Will, please send to Jen Hanenburg at: jen.hanenburg@associationofhorizon.org
