2024 Summer Camp Application

When completing your camp application, please be sure to read each question carefully and answer each question completely and truthfully.

1. Have the following information ready before you apply. Note: you cannot save and return to the application; please allow 15-20 minutes to apply:
   1. Insurance Information (Private insurance, public aid, Medicare, and/or Medicaid numbers.)
   2. Medical History
   3. Emergency Contact, Primary Care Provider Contact, and
   4. References (first time applicants only)
2. Required Paperwork/Follow-Up Forms – Submit the following paperwork as part of your application, or via e-mail or US mail to the contact below:
   1. 1. Physical form dated July 15, 2024 or later (required; other forms such as school physicals are accepted)
      2. Photo of your insurance card(s) (required)
3. Optional Paperwork/Follow-Up Forms – If you wish, you may submit the following paperwork as part of your application, or via e-mail or US mail to the contact below:
   1. 1. Covid Vaccination Card (Optional)
      2. Medication list (Optional, Camper/Participants only)
      3. Any other documents you wish to provide, such as Power of Attorney information, or Living Will.
