2011 Sponsorship/Donation Form
The Association of Horizon, Inc. will be hosting its 20th Annual Summer Camp Program July 24-30, 2011. The focus of Horizon’s summer camp is to provide an opportunity for people with and without physical disabilities to interact and challenge themselves by working together and learning from each other. Each adult with a disability (camper) is paired with at least one person without a disability (volunteer) for the entire week. Often times this week of camp is the first and only time that the camper is independent, without the constant attention of their parents or caregivers. This unbelievable feeling of independence is then carried over into their everyday lives.
Horizon spends $800 per adult with a disability for this unique camp. This includes campsite and medical equipment rentals, insurance, and food. In order for Horizon to keep this free of charge for all of the participants we depend on generous companies and individuals; it’s our hope that you will join them. We are asking for donations and/or sponsorships (all tax deductible) for our summer camp.
SPONSORSHIP LEVELS
PLATINUM — $1,500
This donation will get your name/logo on our banner that will be on display for the entire week of camp, and you will receive a half-page advertisement in our newsletter. In addition, you will receive all the benefits of an $800 sponsorship.
GOLD — $800
This donation enables you to sponsor an entire week of camp for a camper/volunteer pair. You will receive a plaque that can be displayed in your place of business thanking you for your wonderful contribution. In addition, you will receive all the benefits of a $500 sponsorship.
Silver — $500
Your name and/or logo will be printed on the back of our camp T-shirt. This could potentially show thousands of potential customers that your company has generously donated to our camp. In addition, your company will receive all the benefits of a $250 level sponsorship.
Bronze — $250
Receive a half page advertisement in our event program that will be handed out to our entire camp population. In addition, a special thank you will be given to you in our newsletter that reaches over 1,200 potential customers.
If a sponsorship does not interest you, we will gladly accept donations of any amount or in-kind donation to be used directly to help support the 2011 summer camp.
