We are already hard at work on Horizon’s 2008 Summer Camp! Here are all the forms you need to fill out to be accepted to camp. Everyone is required to have a physical.

Once Horizon receives your application (via mail or online) you will be notified that we have your forms. New in 2008 you can check the Horizon website. You will be listed with the forms you have in and have been sent. This list will be updated every week. Our hope is that this list will accomplish two things, 1) you will be able to see that your application has been received and it is being processed, and 2) you can see if your friends have sent in their applications yet, and if they have not you can bug them to get them in. If accepted, you will receive an acceptance package with all the details you will need for camp. All new applicants will be interviewed. New applicants are required to attend the orientation in June. If you do not hear from us, please contact the person responsible for getting the forms.

We love to hear people say they are coming to camp, but what we really need to see is their application. The application deadline is set at May 15th, this is necessary so the recruitment committee can have time to pair up campers with attendants. Campers are not sent an acceptance packet until Horizon is sure that we have enough attendants to pair up with campers. Horizon is offering an incentive to you. If Horizon receives your application by the deadline, your name will be entered into a raffle drawing.

We are looking for new people to get involved. You can help by passing on the application form to:

  • Adults with a physical disabilities
  • Enthusiastic attendants - 16 years and older
  • A school, church or work place where we can recruit new people.

  • Without attendants it would be impossible to be the organization we are today! Attendants can fulfill their high school service hour requirements and it looks great on college resumes. Help the recruitment committee by suggesting a school, church or work place where we can get more new people.

    Space is limited. All applicants will be taken on a first come, first serve basis. (As always, full time applicants will be placed first.) No one will be considered for acceptance without All paperwork! We will no longer hold spots based on verbal promises!

  • Fill out the form below (Please complete all sections. Incomplete applications will result in a delay in processing) and print and return the Camp Waiver OR print and return the camp application
  • Print and return the Medical Form
  • Print and return Health Questionnaire (campers only)
  • Mail copies of Health Insurance card. Public Aid care, SSDI card, Durable Power of Attorney for Health Care, DNR orders, etc.
  • All forms are due by June 1st. We cannot send out your acceptance package until all paper work has been receieved.

    ALL APPLICANTS: CAMPER AND ATTENDANTS

    Camp runs from Sunday, July 13th to Saturday, July 19th, 2008

    Those who are able to attend for the full week will be placed first!

    We will do our best, but due to certain circumstances such as limited number of attendants we have to place people where needed. Are you aware of any physical or mental disability that may limit your performance? If yes, please explain how we may accommodate you.

    FOOD OPTIONS

    TRANSPORTATION

    We may be able to help you find transportation to/from camp or may assist with matching people for ride sharing. Your help and cooperation is appreciated.

    CAMPERS ONLY

    The cost per person in 2007 was $85. Checks/money orders made out to Association of Horizon, Inc. are due in June after the bus contact calls those interested. Pick-up / drop-off locations to be determined in June, usually within 30 minutes of your house.
