SUMMER CAMP APPLICATION

Application Deadline for Adults with Disabilities is May 1st.
Application Deadline for Attendants is May 1st, but will except until full.
Space is limited. All applicants will be taken on a first come, first serve basis. Full time applicants will be placed first.
Application Requirements
- Please fill out your application completely. Applications that are not filled out completely will be delayed in the acceptance process. We cannot send camper acceptance packets until we are sure that there are enough attendants to pair with each camper.
- To complete your application online, click Attendant Application or Camper Application
- To complete a paper application: download 2012 Camper Application or 2012 Attendant Application, print and mail in
- For online applications printed and return the 2012 Waiver Form
- All applicants print and return the 2012 Physical Form
- Horseback riding will be available this year. In order to ride, we need a 2012 Horseback Waiver signed and returned
- All applicants must mail copies of Health Insurance card, Public Aid card, SSDI card, Durable Power of Attorney for Health Care, DNR orders if applicable
- If you would like an application in the mail, please send a self-addressed stamped envelope to Jen (campers) or Sheila (attendants).
Acceptance packets may be delayed if paperwork is not received.

If there are any changes to your submitted application (for example, a new phone number, new medication, new medical equipment needed/used, or new medical daily procedures), it is your responsibility to contact a committee member to make us aware of the change. Failure to disclose all pertinent medical information can result in removal from camp or denial of future applications.
