CAMPER/PARTICIPANT applications are due by May 1; a waitlist will begin after capacity is reached.
VOLUNTEER applications are due by June 1.
Physicals, insurance cards, and additional forms are due by June 10.

Before Applying, Please Read:
When completing your camp application, please be sure to read each question carefully and answer each question completely and truthfully.
- Have the following information ready before you apply. Note: you cannot save and return to the application, you MUST apply in one sitting, please allow 15-20 minutes to apply:
- Insurance Information (Private insurance, public aid, Medicare, and/or Medicaid numbers.)
- Medical History
- Emergency Contact, Primary Care Provider Contact, and
- References (first time applicants only)
- Required Paperwork/Follow-Up Forms – After you submit your application, you will receive a confirmation email with the links to upload the following:
- Physical form (required)
- Photo of your insurance card (required)
- Optional Paperwork/Follow-Up Forms – After you submit your application, you will receive a confirmation email with the links to upload the following:
- Covid Vaccination Card (Optional)
- Medication list (Optional, Camper/Participants only)
- For any other documents you wish to provide, such as Power of Attorney information, or Living Will, please send to Jen Hanenburg at: jen.hanenburg@associationofhorizon.org

NOTE: If there are any changes after you have submitted your application (for example, a new phone number, new medication, new medical equipment needed/used, or new medical daily procedures), it is your responsibility to notify us.
NOTE: Failure to disclose all pertinent medical information can result in removal from camp or denial of future applications.
